SUMMARY We are looking for Paraprofessionals to work with behavioral health and similar service needs. Follows treatment team recommendations in providing daily living support to young adults. Direct care staff. Must have own, insured vehicle to use for this position.
CNA and LPN experience a plus! ESSENTIAL DUTIES AND RESPONSIBILITIES • Model best practices in carrying out Paraprofessional responsibilities. • Reads every day and follows each individual’s Positive Program Plan (PPP) schedule. • Ensure young adults’ medical health issues are addressed.
This may include scheduling doctors’ appointments, providing transportation to appointment, following through on doctor’s orders and communicating all pertinent health issues to supervisor. Monitors, observes and prompts young adults’ self- administration of medication. • Assist young adults in daily activities including daily hygiene functions. • Implement behavioral goals, teaching strategies, monitoring procedures, exercise programs, ARC trauma informed practices and protocols and other team recommended programs and procedures.
- Assist in the completion of assessments. • Model positive adult roles in the community and foster community belonging through young adult centered appropriate activities • Provide active treatment to young adults, as outlined in the Individual Plan. • Provide meaningful community experiences to young adults. • Maintains specified level of supervision of young adults in the community and at home.
- Documents case notes in ETO, communication logs, programs, incident reports, abuse and neglect reports, safety reports, fire drills, time sheets, mileage forms and other required documentation timely and accurately. • Implement monitoring protocols. • Implement Physical/Psychological Management Training, CPR, and First Aid as required • Maintain a safe and clean work environment. This includes housekeeping such as dishes, floors, countertops, bathrooms, kitchen areas, simple yard work, and other areas.
- Pre-vocational/vocational skill development duties are included. These may include active participating in Life Skills Training, assisting with job applications, interviews, job coaching, etc. • Represent the values, mission and vision of the organization. Mission: To provide residential, employment, support, referral, and advocacy services to individuals with disabilities and people with similar service needs to assist them in exercising their human rights as citizens and contributing members of society.
- Process skills which are required to plan and implement services for individuals with complex trauma histories and/or mental health disorders. EDUCATION and/or EXPERIENCE High school diploma or general education degree (GED); 3 to 6 months related experience and /or training, or human service training certificate. Experience using basic computer programs. CERTIFICATES, LICENSES, REGISTRATIONS • Valid Connecticut Driver’s License • Ability to obtain the following licenses/certifications: CPR and First Aid certification, as required by program site.
